Direct Deposit Allocation
- 1a First Account Routing Number The nine-digit routing number for the first bank account where you want part of your refund deposited.
- 1b First Account Number Your personal account number at the bank identified on line 1a.
- 1c Amount to First Account The dollar amount you want deposited into the first bank account identified on lines 1a and 1b.
- 2c Amount to Second Account The dollar amount you want deposited into a second bank account.
- 3c Amount to Third Account The dollar amount you want deposited into a third bank account.
